What Is Endometrial Thickness, and Why Does It Matter for Pregnancy?

Endometrial thickness refers to the thickness of the lining of your uterus—the soft, blood-rich layer that builds up each month to prepare for embryo implantation. Think of it as the soil, and the embryo as the seed. Even a healthy embryo may find it harder to implant when the endometrial lining is too thin or not sufficiently receptive.

A healthy endometrium provides a suitable environment for embryo implantation and supports the early stages of pregnancy and placenta development through an adequate blood supply. That’s why endometrial thickness is carefully assessed before embryo transfer, particularly during IVF treatment. How Endometrial Thickness Changes During the Cycle

Your endometrium isn’t a fixed thickness; it grows and sheds every month, which is why when it’s measured matters. Just after your period, it’s thin, having recently shed. Through the first half of the cycle, rising estrogen builds it up, thickening it steadily. Around ovulation, it typically reaches roughly 8–12 mm and takes on its receptive triple-line look. After ovulation, progesterone takes over and matures the lining, making it softer and more nutrient-rich the ideal surface for an embryo.

In IVF, medications simply mimic this natural rhythm: estrogen builds the lining, and progesterone prepares it, all timed to line up with the embryo’s arrival. That’s why your lining is measured at specific points around trigger time in a fresh cycle, or during preparation for a frozen transfer.

Why the trilaminar pattern matters as much as the number

You’ll often hear the word “trilaminar,” or “triple-line.” It describes how a receptive lining looks on ultrasound near ovulation: three distinct layers, like a faint sandwich. It’s a reassuring sign that the endometrium has developed properly under estrogen.

This is why an experienced doctor never reads thickness in isolation. A lining that’s a good number and trilaminar is ideal. A lining that hits the number but shows a poor pattern may be less favourable than the millimetres alone suggest. Pattern and thickness are read together, which is another reason a single figure on your report doesn’t tell the whole tale.

What causes a thin endometrium?

If your lining is stubbornly thin, there’s almost always a reason, and finding it is the first step to fixing it. Common causes include low estrogen levels or a naturally thinner lining with age; Asherman’s syndrome, where scar tissue (adhesions) forms inside the uterus, usually after a D&C, surgery, or infection; chronic endometritis, a low-grade inflammation of the lining; and genital tuberculosis, which is an important and under-recognised cause of a damaged, thin lining in India in particular. Poor blood flow to the uterus, previous pelvic radiation, and the anti-estrogen effect of medications like clomiphene can also play a role.

What if my lining is too thick?

Far fewer people worry about this, and rightly so a thicker lining is usually a good sign, not a bad one. A generously thick, trilaminar lining generally means your uterus has responded well. Occasionally a very thick lining (above roughly 14–15 mm) prompts your doctor to take a closer look, just to rule out something like a polyp, but in most cases it’s perfectly fine and no cause for alarm.
